 » – Louis PasteurBorderline personality disorder (BPD) is a serious mental illness that centers on the inability to manage emotions effectively. Other disorders, such as depression, anxiety disorders, eating disorders, substance abuse and other personality disorders can often exist along with BPD. It can cause intense mood swings, impulsive behaviors, and severe problems with self-worth. It can lead to troubled relationships in every area of a person’s life.

Impact of Obamacare

The public exchanges like the SHOP (Small business Health Option Program) exchange for small businesses or the federal or state exchanges for individual health plans are operated by the government. Healthcare.gov will direct you to the appropriate exchange. However, the actual health plans are operated by private insurance companies, which include many of the leading carriers such as Blue Cross Blue Shield, United Healthcare, Aetna, Kaiser Permanente, Humana, Cigna, Coventry, etc.

The Obama Administration’s budget proposal for 2016 begins to apply price controls and shortened patent protection to reduce the enormous cost of pharmaceuticals in the U. S. The Administration wants to reduce intellectual-property patent protection for biologic therapies to seven years from 12, and to reduce payment rates for hospital-administered medicines. Medicaid « rebates » – which require drug makers to offer the government discounts from average drug prices – would be extended into the Medicare prescription benefit as well. These changes are expected to alleviate the growth in taxpayer funded health care spending, but will also pressure margins across the pharmaceutical industry.

Though Obamacare extends coverage behavioral health patients, various loopholes in the health care law at this time have kept people from requesting mental health care. Some states haven’t expanded Medicaid, leaving about 5 million in a coverage gap. In other cases, patients aren’t even aware of the benefits they can get with their new health insurance.

Scope of Mental Health Issues in the U.S.

Mental health is one of the most common health care issues, affecting as many as 1 in 4 adults each year. The federal Substance Abuse and Mental Health Services Administration found in a 2013 report that 9.6 million adults reported having a serious mental illness, such as major depression, schizophrenia, bipolar disorder, obsessive compulsive disorder, panic disorder, post-traumatic stress disorder and borderline personality disorder. People with mental illness require an individualized treatment plan that could require medications, therapy and participation in peer support groups.

People who don’t have health insurance are more likely to put off medical care or to skip it altogether. Untreated mental illness can cause someone to become unemployed or homeless, or to turn to substance abuse and even suicide.

Obamacare updated the Mental Health Parity and Addiction Equity Act of 2008, which requires insurance companies to offer the same amount of coverage for mental health and substance use disorders as they would for medical or surgical procedures. A plan that covers mental health benefits must cover medications, emergency care and outpatient and inpatient services.

When the law was passed, about 20 percent of Americans did not have coverage for mental health services, and one-third of those covered in the individual market did not have coverage for substance abuse. It’s difficult to quantify how many therapy sessions a person needs, but chemotherapy treatments are more strictly defined. Insurance companies tend to both limit the number of sessions they will cover, as well as the amount they will reimburse. For this reason, many psychiatrists do not accept private insurance or Medicaid.

Under Obamacare, plans in the marketplace are required to cover 10 Essential Health Benefits, including mental health and substance use disorders. The law does not specify which particular services should be covered within each category. States therefore vary on what they require the marketplaces to include, or what they choose to include in Medicaid programs.

Borderline Personality Disorder

Most of the time, signs of BPD first appear in childhood. But problems often don’t start until early adulthood. Treatment can be hard, and getting better can take years. Problems with emotions and behaviors are hard to improve. But with treatment, most people with severe symptoms do get better over time.

Experts don’t know exactly what causes borderline personality disorder. Problems with chemicals in the brain that help control moods may play a role. It also seems to run in families. Often, people who get it faced some kind of childhood trauma such as abuse, neglect, or the death of a parent. The risk is higher when people who had childhood trauma also have problems coping with anxiety or stress.

The most common symptoms of BPD include:
Intense emotions and mood swings.
Harmful, impulsive behaviors. These may include things like substance abuse, binge eating, out-of-control spending, risky sexual behavior, and reckless driving.
Relationship problems. You may see others as either « good » or « bad » and may shift from one view to the other suddenly, for minor reasons. This can make relationships very difficult.
Low self-worth.
A frantic fear of being left alone (abandoned). This fear may lead to frantic attempts to hold on to those around you. Or it may cause you to reject others before they can reject you.
Aggressive behavior.

Other symptoms may include:
Feeling empty inside.
Problems with anger, such as violent temper tantrums.
Hurting oneself, such as cutting or burning oneself.
Suicide attempts and suicidal thoughts.
Times when you feel paranoid or lose a sense of reality (psychosis).

It’s easy to confuse this disorder with other mental illnesses. And, they may overlap. Borderline personality disorder can be hard to treat. It’s common for symptoms to return. And many people with the disorder have troubled relationships with their counselors and doctors.

Treatment for BPD may include:
Counseling and therapy. It’s important to find a counselor you can build a stable relationship with. This can be hard, because your condition may cause you to see your counselor as caring one minute and cruel the next, especially when he or she asks you to try to change a behavior. Try to find a counselor who has special training in dialectical behavioral therapy (DBT) to treat this disorder.
Medicines, such as antidepressants, mood stabilizers, and antipsychotics. In combination with counseling or therapy, they may be helpful in treating symptoms of borderline personality disorder.
Healthy habits, such as getting enough sleep, eating healthy foods, getting regular exercise, and avoiding alcohol and drugs. These habits can help reduce stress and anxiety. And they can help make your symptoms less severe and less frequent.
